Riverpark Dental will provide documents or information in an accessible format or with communication supports upon request. We will consult with you to determine the best format or support, based on your needs and what is feasible for the clinic.
Examples include large print, accessible PDF, plain text, audio recordings, email instead of hard copy, captioned videos, or reading information aloud. Documents will be provided in a timely manner and at no additional cost beyond regular charges to others.
We welcome individuals with disabilities and their service animals. Service animals are permitted in areas that are open to the public and third parties.
If it is not obvious that the animal is a service animal, staff may ask for documentation from a regulated health professional confirming the need for the service animal. Please ensure service animals are clearly identified when possible, for example with an identification card, harness, or jacket.
Owners are responsible for the care, supervision, and control of their service animal while on the premises, including feeding and bathroom breaks. Service animals should be kept on an appropriate lead unless otherwise required for the animal's role.
